Our professional nursing team provides compassionate, attentive care to support each participant’s health and well-being. Our Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) provide health monitoring, vital sign checks, medication management, chronic condition management, health education, and other nursing services. Working closely with physicians and our multidisciplinary team, we provide individualized care and support to help our participants stay healthy, safe, and comfortable each day.

Our social work team is here to listen, support, and care for each participant and their family. We provide case management, emotional and psychosocial support, community resource connections, benefits assistance, family support, and care coordination based on each participant’s individual needs. Working closely with our multidisciplinary team, we help our participants feel understood, respected, supported, and connected throughout their journey with us.
我们的社工团队用耐心倾听、用关怀陪伴，为长辈及家属提供贴心的支持。社工会定期关注长辈的情绪、社交及生活需求，提供个案管理、心理与情绪支持、社区资源连接、福利协助及家庭沟通等服务，并与跨专业团队共同制定适合每位长辈的照护计划。我们希望每一位长辈在这里不仅得到专业的照顾，更能感受到被理解、被尊重和被关心。
Our rehabilitation team provides Physical Therapy (PT) and Occupational Therapy (OT) services to help participants maintain and improve their mobility, strength, balance, and ability to perform daily activities. Based on each participant’s individual needs, our therapists provide personalized exercises and training, including strength and balance training, mobility and gait training, range-of-motion exercises, daily living skills, and fall prevention. With professional guidance, patience, and encouragement, we help our participants stay active, confident, and as independent as possible.
我们的复健团队通过专业的物理治疗（PT）与职能治疗（OT），帮助长辈维持和改善身体机能、行动能力与日常生活能力。根据每位长辈的身体状况和个人需求，治疗师提供个性化的训练，包括肌力与平衡训练、步态与行动训练、关节活动、日常生活技能训练及跌倒预防等。我们希望通过耐心的指导与持续的鼓励，帮助长辈保持活力与自信，尽可能安全、独立地享受每一天。

We offer a variety of engaging daily activities designed to keep our participants active, connected, and enjoying each day. Activities may include dancing, music and singing, arts and crafts, games, gentle exercise, holiday celebrations, monthly birthday party, and other recreational programs tailored to participants’ interests and abilities. Through meaningful participation and social connection, we encourage physical and emotional well-being while creating a warm community filled with friendship, laughter, and joy.
我们精心设计丰富多彩的日常活动，让长辈在轻松愉快的氛围中保持活力、结交朋友、享受每一天。活动内容包括舞蹈、音乐与歌唱、手工、书法与绘画、益智游戏、轻度运动、节庆活动、生日会及各种兴趣活动等，并根据长辈的能力与兴趣进行适当安排。我们鼓励每一位长辈积极参与、彼此交流，在欢笑与陪伴中促进身心健康，感受属于这里的快乐与温暖。

We provide safe, convenient, and comfortable transportation services to make each participant’s journey to and from the center easier and worry-free. Our drivers provide scheduled transportation between participants’ homes and the center, offering assistance with boarding and exiting the vehicle as needed while paying close attention to each participant’s mobility and safety needs. With caring and reliable transportation, we help participants enjoy their day at the center while giving families greater peace of mind.

Our goal is to make Chino Hills Adult Day Health Care Center more than a place to receive care — we want it to feel like a welcoming community where seniors feel valued, connected, and at home.
