Chino Hills Adult Day Health Care Center was established in 2023 to provide professional adult day health care services to older adults and individuals with care needs in our community. Located in Chino Hills, California, our center operates in accordance with applicable California Adult Day Health Care requirements and maintains the appropriate California Department of Public Health (CDPH) licensure and California Department of Aging (CDA) certification/approval to provide comprehensive adult day health care services.

Since our founding, we have remained committed to a participant-centered approach to care. We believe quality care is about more than supporting physical health — it is also about promoting emotional well-being, social connection, independence, dignity, and a better quality of life.

 

Our multidisciplinary team includes nursing professionals, social workers, physical and occupational therapy professionals, activity staff, and other dedicated care providers. Working together, our team develops individualized care and support based on each participant’s health conditions, abilities, and personal needs. Our services include professional nursing care, health monitoring, social work services, rehabilitation, engaging activities, nutritious meals, and transportation.

 

At Chino Hills Adult Day Health Care Center, our goal is to create more than a place to receive care. We strive to provide a warm and welcoming community where every participant feels safe, respected, valued, and connected.
